Access: Through the App mothers will have at their fingertips 24/7 access to care, advocacy, education and engagement, and links to supportive services to address SDoH, to promote healthy pregnancies and birth outcomes.

Connected Care: Mothers will be issued a Community Care Kit to include eHealth wearables to monitor blood pressure, heart, SPO2, and sleep; Wi-Fi enabled weight scale; glucoseometers as needed.

Cultural Competency: 24/7 culturally competent remote monitoring, education and engagement, and links to supportive services to address SDoH to promote healthy pregnancies and birth outcomes.

Mother Monitoring For Prevention: If an expected mother’s vitals fall outside pre-configured parameters, a trigger response protocol is initiated, facilitating triage and real-time intervention.

RPM in non-obstetrical settings has been shown to be cost-effective and cost-saving by reducing hospital readmissions, length of stay, medications administered, and laboratory studies performed. It has proven to be a powerful and highly effective tool to improve access to care for expectant mothers with gestational hypertension, gestational diabetes, and other conditions which require regular readings of physiological data. RPM provides an uncomplicated way to gather such data without necessitating office visits. It has the potential to substantially benefit health management for high risk pregnancies, and contributes to significant reductions in the cost of care, face to face visits, low birth rate, and NICU admissions.  Research also shows that telehealth program prompts patients to have more prenatal care visits, with improved utilization of prenatal care services.
