WHO WE ARE
We are Community Care Connection, Inc., a nonprofit, 501c3 remote patient monitoring company committed to improving maternal and infant health outcomes, staffed by culturally competent nurse-doulas, community health workers, and telehealth professionals. We are focused on advancing maternal health equity through a combination of culturally competent programming, remote mother monitoring, and education and engagement to improve health literacy. We believe these attributes will substantially improve maternal and infant health outcomes by effectively bringing caring to where they live.

OUR MODEL
For years, the United States has been grappling with a concerning maternal health crisis, particularly among underserved urban and rural populations. Alarmingly high rates of maternal mortality and morbidity persist, shining a spotlight on the urgent need for targeted interventions to address this pressing issue. In response, a myriad of initiatives have emerged, each aiming to alleviate the burden on vulnerable communities and ensure equitable access to quality maternal health care. Among these initiatives stands the Community-based Health Equity Model built on four pillars, a beacon of hope in the fight for maternal health equity. We work with Health Equities RPM (“H.E.R.”) and its Health Equity Partners to implement a maternal health equity initiative based on the Model. The Four Pillars form the bedrock of a program meticulously crafted to not only address the immediate healthcare needs of expectant mothers, but also foster enduring relationships built on mutual respect and understanding.
